You get a health insurance card when you are registered with a CPR number as a resident of Denmark.

The yellow health insurance card (sundhedskort) looks like this:

It contains the name, address, and telephone number of your GP as well as your CPR number, and your name and address. 

It is advisable that you always carry the card as you will need to scan the card when you see your GP or attend other healthcare professionals/locations.

If you have lost your health insurance card, or if it is broken, you can order a new one online. You order a health insurance card using your MitID. 

If you change your address the card is free of charge.

 You may order a health insurance card if you are over the age of 15. Parents may order a health insurance card for children living at home until they have turned 18. 

Get the health insurance card as an app

When you have a CPR number you may download the yellow health insurance card as an app. Search for "sundhedskortet" where you download your apps. 

A new health insurance card costs 220 kroner (2023) if:

  • The health insurance card is lost
  • The health insurance card was issued less than four years ago and is broken
  • You wish to change your doctor or health insurance group
  • You change your name

A new health insurance card is free of charge if:

  • The card's magnetic stripe doesn't work, but the card is otherwise intact
  • The health insurance card is broken and was issued over four years ago
  • You need a new health insurance card as you are moving
  • Your general practitioner (family doctor) is no longer available
  • You change your name because you have been married (within the first three months after the wedding). 

Your health insurance card is valid as long as you are registered with a CPR number as a resident of Denmark and the information on the card still applies (e.g. name, address, general practitioner).

If you do not have your name on your postbox, the card will be returned to Citizen Service at Dokk1.

Come by Citizen Service to pick it up. We will keep the yellow card for up to 6 months. If we no longer have your card, you will need to pay for a new one.

 

It is very important that your name is correctly spelled. Call Citizen Service at +45 89 40 10 30. We will make sure that you receive a new card with the correct spelling of your name.

The Health Insurance Card app is a voluntary digital supplement to your physical health insurance card.

With this app, you will always have your health insurance card close at hand.

Just as is the case for your physical health insurance card, the Health Insurance Card app will enable you to document your entitlement to receive public healthcare benefits in Denmark.

The health services are obligated to receive you if you bring the Health Insurance Card app.

The Health Insurance Card app fulfills the function of a valid health insurance card and is used in the same way as the physical health insurance card.

You can, for instance, use the Health Insurance Card app at your GP, the pharmacy, the hospital, or the dentist by scanning the barcode of the Health Insurance Card app in the same way as you scan the barcode of your physical health insurance card.

As the Health-insurance Card app is a voluntary supplement to your physical health insurance card, you must hold on to your physical health insurance card – also after having downloaded the app.

If you have children, the Health Insurance Card app will automatically give access to your children’s health insurance cards, until the children reach the age of 15.

Your data will automatically be updated in the app, e.g. in case of a change of your last name, address, or GP.

You will still receive a new health insurance card in the mail if you, for instance, move to a new address or change GP.

The creation of your health insurance card in the Health Insurance Card app, presupposes that you:

  • live in Denmark
  • have NemID/MitID
  • belong in healthcare insurance group 1 or 2

The Health Insurance Card app is not available to citizens with special health insurance cards (særligt sundhedskort).

The Health Insurance Card app, which can be downloaded to up to three devices, is available for both iPhone and Android phones:

  • iPhone with operating system iOS 13 or later
  • Android phone with operating system Android 8.0 (Oreo) or later.
