The yellow Danish health insurance card (sundhedskort)
The yellow health insurance card (sundhedskort) is your documentation that you are entitled to the services offered under the Danish national health insurance scheme. If your heath insurance card is lost or broken you can order a new card.
About the card
You get a health insurance card when you register with a CPR number as a resident of Denmark.
The yellow health insurance card (sundhedskort) looks like this:
It contains the name, address, and telephone number of your General Practitioner (GP / family doctor), your CPR number, and your name and address.
It is advisable that you always carry the card, as you will need to scan it when you see your GP or visit other healthcare professionals/locations.
Do you need a new health insurance card?
If your health insurance card is lost or broken, you can order a new one online using your MitID.
If v free of charge.
You may order a health insurance card if you are over 15. Parents may order a health insurance card for their children living at home until they turn 18.
A new health insurance card costs 225 kroner (2024) if:
- The health insurance card is lost
- The health insurance card was issued less than four years ago and is broken
- You wish to change your doctor or health insurance group
- You change your name
A new health insurance card is free of charge if:
- The card's magnetic stripe doesn't work, but the card is otherwise intact
- The health insurance card is broken and was issued over four years ago
- You need a new health insurance card as you are moving
- Your general practitioner (family doctor) is no longer available
- You change your name because you have been married (within the first three months after the wedding).
Your health insurance card is valid as long as you are registered with a CPR number as a resident of Denmark and the information on the card still applies (for instance, name, address, general practitioner).
The app is a voluntary digital alternative to your physical health insurance card, and it is just as valid as your physical card.
Look for the app "Sundhedskortet", where you usually find your apps.

Please keep your physical health insurance card safe if you use the app instead of the physical card.
Please note that the app is not available for citizens with special health insurance cards (særligt sundhedskort).
If you need help ordering a new health insurance card, please call Citizen Service (Borgerservice) at 89 40 22 22.
If you do not have your name on your postbox, the card will be returned to Citizen Service (Borgerservice) at Dokk1. We will contact you if the card is returned to Citizen Service.
Come by Citizen Service to pick it up. We will keep the yellow card for up to 6 months. You must pay for a new one if we no longer have your card.
Your name must be correctly spelled. Call the National Registration Office (Folkeregistret), part of Citizen Service (Borgerservice), at +45 89 40 10 30. We will ensure you receive a new card with your name spelt correctly.

You must book an appointment yourself online for a change of foreign driver's license, MitID, and the issuing of a CPR number.
